Pragmatic Free Trial Meta

%ED%94%84%EB%9D%BC%EA%B7%B8%EB%A7%88%ED%Pragmatic Free Trial Meta is a free and non-commercial open data platform and infrastructure that supports research on pragmatic trials. It collects and distributes cleaned trial data, ratings and evaluations using PRECIS-2. This allows for a variety of meta-epidemiological analyses that evaluate the effects of treatment across trials with different levels of pragmatism.

Background

Pragmatic trials provide evidence from the real world that can be used to make clinical decisions. However, the usage of the term "pragmatic" is inconsistent and its definition as well as assessment requires further clarification. Pragmatic trials are designed to inform clinical practices and policy decisions, not to prove a physiological or clinical hypothesis. A pragmatic study should strive to be as close as is possible to actual clinical practices, including recruitment of participants, setting, design, implementation and delivery of interventions, determining and analysis results, as well as primary analysis. This is a key distinction from explanatory trials (as described by Schwartz and Lellouch1) that are designed to provide more thorough confirmation of an idea.

Results

Although the definition of pragmatism may not require that all trials be 100 percent pragmatic, there are some advantages to including pragmatic components in clinical trials. These include:

Increasing sensitivity to real-world issues, reducing cost and size of the study and allowing the study results to be faster implemented into clinical practice (by including patients who are routinely treated). However, pragmatic trials have their disadvantages. The right amount of heterogeneity, for example could help a study expand its findings to different patients or settings. Many studies have attempted categorize pragmatic trials using various definitions and scoring methods. It is important to remember that a pragmatic trial does not necessarily mean a low-quality trial, and indeed there is a growing number of clinical trials (as defined by MEDLINE search, but this is neither sensitive nor specific) which use the word 'pragmatic' in their abstract or title. The use of these words in abstracts and titles could indicate a greater understanding of the importance of pragmatism, but it is unclear whether this is reflected in the content of the articles.

Conclusions

As the value of real-world evidence grows commonplace and pragmatic trials have gained momentum in research. They are randomized trials that compare real world care alternatives to clinical trials in development. They include patient populations more closely resembling those treated in regular care. This approach can overcome the limitations of observational research, for example, the biases that are associated with the reliance on volunteers, and the lack of the coding differences in national registry.

Pragmatic trials offer other advantages, like the ability to leverage existing data sources and a greater probability of detecting meaningful distinctions from traditional trials. However, these tests could still have limitations which undermine their reliability and generalizability. Participation rates in some trials could be lower than expected due to the healthy-volunteering effect, financial incentives or competition from other research studies. Many pragmatic trials are also restricted by the necessity to recruit participants in a timely manner. Some pragmatic trials also lack controls to ensure that observed variations aren't due to biases that occur during the trial.

The authors of the Pragmatic Free Trial Meta identified 48 RCTs self-labeled as pragmatic and that were published up to 2022.
